Fukushima leaks: radioactive water overflows tank, -BBC

Not so much that a worker got his sums wrong, but more so that those massive tanks were built on ground that wasn’t level. Was the ground not prepared? dug out, compacted crushed rock, a concrete raft foundation/base put in place? If that’s the case then the failure of those storage tanks is going to be a major problem.
